Who Can Participate
New Yorkers aged 18 or older from all five boroughs of New York City – Manhattan, Brooklyn, Queens, Bronx, and Staten Island – are randomly selected to take part in the mail-based survey.
The survey can be taken in English, Spanish, Chinese (Mandarin and Cantonese), Russian, and Haitian Creole.
The survey can only be completed by one person per household over the age of 18.
If multiple people in one household are selected to participate, only the household member with an upcoming birthday will be permitted to take the survey.
